Getting on top of it or doing it on it is a verbal periphrasis that is used as a euphemism to indicate that someone does not dominate the sphincters, no longer controls their elemental and physical springs, both in urications and defecations. If we are allowed vulgarity, it is pissed or shit (even both functions) without going to the toilet or lowering your intimate garments.
