This phrase is preceded by a verb such as wanting, asking, pretending, waiting, demanding , . . . and it's used to recriminate anyone who wants to get something, and if he gets it, he'll take advantage to request something else. The original story tells about someone asking for a topical remedy (medicine) to put on a wound, and then the rag to apply it.
