It is an expression that is used to describe a degree of extreme poverty, that you are without economic resources or that have been lost in a short time. It is a reference to nudity, an exaggeration for having lost even clothes and having to use your hands to cover the pudendal parts. In the dictionary there are other poorly worded versions, such as "with a rag behind and another in front in front", "with one hand back and another in front with one hand in front and another behind".
